General Office & Policies
All patients who wish to be seen must make an appointment. If you need to cancel an appointment, please notify us at least 24 hours in advance so we can fill your time window. There is a genuine effort made on our part to see every patient at their scheduled time and therefore we ask that you be punctual. All office staff will be able to answer most of your questions asked over the telephone. If needed, our provider will be consulted and the answer then relayed to you. Any calls that the provider must return personally if not returned during the day, will be made after office hours. It is necessary that you keep track of your medications and have them refilled before you run out. Calls to refill prescriptions should be made in the morning and you can expect it to take 24 hours to provide. Please do not wait until your prescription runs out to place the call to refill. Your prescriptions cannot be refilled after office hours or on the weekend. It is our policy to regulate prescription refills made over the telephone to within 2 month year of the most recent visit regarding oral medication and 3 months for the most current topic related medications.
Late arrivals may be required to reschedule. Unless you arrive more than 30 minutes late, there will not be a cancellation fee.
What you should bring
Please remember to bring your photo ID and insurance card to your appointment. In order to file an insurance claim, we might also require your social security number. If you fail to provide the above information, we might charge you the full cost of the appointment and any procedures performed at the time of service.
A parent or legal guardian must be in attendance during the initial appointment and any appointments involving invasive treatments. For all appointments, minor children must have a parent or any authorized person of age 18 or older present.
Transfers From Other Doctors
If you are transferring to our practice from another physician, please have all of your medical records faxed over to our office prior to your visit. This will eliminate the possibility of any confusion regarding previous treatments.
Biopsy Call Backs
Patients who have had any biopsy results performed will be called back within 10-14 business days of the biopsy. If a patient has not received a call regarding their results after two weeks, please do not hesitate to call our office to request your results. Unless a result is abnormal, lab results do not usually require a call back.
Prescriptions can be filled during business hours only. Please leave our medical assistants a message, and they will return your call by the following business day.
